                  Thanks Ed – he has made me more optimistic! I wonder if they really can do what he proposes: lots of trials and trying to select people into the right ones, and then I think assessing how they are doing and then maybe switching them. Seems quite complicated amd requires knowledgeable doctors, but was impressed by the presentation and that the future could be better. Also that melanoma seems to lead the way for immunotherapy!!! Thanks very much for sharing

                    Hi Mark, some of the larger hospitals are already doing some of the ideas that Dr. Omid Hamid talked about. In Chicago Dr. Jason Luke is part of the advise trial where at progression they look at the tissue and based on tumor make a decision on next steps, and at MD Anderson smaller trials looking for proof of principle are more common now than just large trials. Precision medicine is happening all over, we just have to look for it and ask our Oncologist what options can they provide for us down the road if our current treatment stops working! I follow Dr. Hamid and Dr. Jason Luke on Twitter and a lot of interesting research pops up, they are part of a large group that like to share new findings and novel trials.                         Hi Ed,

                        Thanks for writing – things for me are OK. Coming up to two years on pembro (with a three month break early on), finally with the addition of tvec six months ago we seem to be turning stable disease into a response. if so i think it is the most delayed response on this forum. I dont want to jinx it though and also prayer and going to church (I hope God) and trying to cycle once a week may have helped.

                        i think my doctors are aware of options if my treatment fails, they seem optimistic that there are new trials out there to help me (i also have the surgery option too as long s no spread). So i think they know the options but dont think they are looking at the tumour and then deciding on the treatment depending on the tumour and i guess how the tumour evolves in response to treatment (is this what dr hamid is suggesting – that would be pretty resource intensive, with biopsies needed maybe and lots of hands on treatment from the doctor).

                        it was good to hear dr hamid and while i dont understand what he was saying it was good to hear him outline plans for patients and that we will have more options than just ipi nivo or pembro.
