Does new lesion on hip mean Keytruda is no longer working for me ?

      Among everything else I'm dealing with they found out the reason my right hip was hurting a lot is because the hip ct yesterday showed a lesion there. He said that was not there on my November petct scan. Since I've been on keytruda since May with a few tumors showing minor growth but no new tumors and other tumors shrinking does this now mean keytruda is failing me so if I get through my current issues I gotta get on a different systemic treatment fast? radiation to hip starts soon to end at same time as rest of radiation.

          I dont think so. Occasionaly, it is an "old" tumor that is going through pseudo-progression before it shrinks. Actually most of the tumors on hodi and Wolcok paper:"Guidelines for evaluation of immune therapy.." http://clincancerres.aacrjournals.org/content/15/23/7412 shows growth and than shrinkage (Fig.1)

                I'm not sure.  I guess Mom is possibly correct, that this is enlargement of not necessarily tumor but infiltrates around it.  However, you have always been proactive and I'd probably be talking to the docs about how they see it and if they think it is time to be looking at the LAG-3 trials, TIL, etc.  I am still holding out for some synergistic effect between your current and upcoming radiation treatments and Keytruda!!!  How is your swallowing?  Are you seeing any improvement there?  Much love, c

                        Thanks Celeste. No swallowing improvement yet. Radiation doc hopes to see some improvement this time next week. I've lost 3 pounds this week so far but I'm trying hard.

                                  I certainly hope it doesn't mean it's stopped working. What does onc say? Praying that all goes well with radiation and Keytruda…get some relief on neck tumors etc…

                                        Artie, I won't go into all the details of what I already posted earlier today, but from what my dad's oncologist at UCSF said, new tumors or significant growth after starting Keytruda does not bode well. When pushed for a prognosis, he said we're talking months in my dad's case, that he's not at the super advanced stage, but that the doctor wouldn't want to postpone a change in treatment for more than a few weeks. 

                                        However, perhaps the radiation would make the difference in how well the lesion will respond to Keytruda now. I'd think that's very possible and is something to ask your doctor about before switching treatments. 

                                        On another note, with the crisis my dad suffered that day, I didn't have a chance to investigate that trial we had talked about. I will still try, though, as things hopefully continue to improve with Dad.
