      Hi All,

       

      I had two moles removed recently and have to do a complete re-excision next week. It's not sitting well with me, so I'm hoping that you all could help me understand this a little better and see if there's anything I should be worried about? See below for pathology report and comments:

       

      Microscopic Description: A. There is elongation, complexity, and bridging of the rete ridges by nested pigmented basal melanocytes with nuclear hyperchromatism, pleomorphism, and focal random, moderate cytologic atypia. Large pleomorphic melanocytes with clear cell areas and large nuclei are noted in the epidermis. The papillary stroma is characterized by fibroplasia, a patchy round cell infiltrate with melanophages and organoid nests of nevomelanocytes.

      B. There are single and focally nested melanocytes at the dermoepidermal junction and nests and strands of ordinary-appearing nevus cells in the dermis. There is interconnection of the rete ridges (bridging) and lateral extension of the epidermal component (shouldering). Some of the melanocytes at the junction have ample dusty cytoplasms and there is random nuclear atypia. There is a sparse, superficial, perivascular lymphohistiocytic infiltrate in there dermis and also lamellar fibroplasia beneath the tips of some of the elongated rete ridges.

      Comment: Complete re-excision is indicated for this lesion.

          Without you including the final diagnosis, we can't really comment.  A microscopic description justifies the final diagnosis.
